  • What is the Bridge Program?
    The Bridge Program provides pain and symptom management as was as emotional support during a period when a patient with a life- limiting condition may still be pursuing active treatment or is not yet ready to elect the Hospice benefit
  • What is Palliative Care?
    Palliative Care benefits patients with a progressive disease for whom the focus is the relief of pain and other physical symptoms while enriching the lives of the patient and family with emotional and social support.
  • Who Will Assist in My Care?
    You, your family, and/or caregivers
    Your physician
    Medical Director
    Skilled Nurses
    Certified Nursing Assistants
    Medical Social Workers
    Other therapies as needed
  • Will My Doctor Continue to Be My Doctor?
    Yes, Zia HomeCare functions as an extension of your attending physician.
  • Who Pays For HomeCare?
    Medicare
    Medicaid
    Managed Care Plans
    Many Long Term Care Insurances
  • Who Is Eligible?
    Patients must meet certain criteria such as:
    Have a need for skilled nursing care
    Be homebound as defined by Medicare*
    *A person who cannot comfortably leave their home without considerable effort, support, and assistance.
  • What If I Decide To Change To Hospice?
    You may transfer to Zia Hospice if you meet Hospice criteria at any time. Our staff is educated to function in both settings so that staff is able to transition with the patient.
